We appreciate your participation in the Instructional Design Team Case Competition 1998, and feel that your experiences, insights, and suggestions would be most valuable to us in better understanding the dynamics of the event and in improving our methods for future Instructional Design Case Events.

Informed Consent Agreement
Project Title: IT Case Event 1998

Please read this consent agreement carefully before you decide to participate in the study.
Purpose of the research study:
The purpose of the study is to further identify the value of multimedia case methods as tools for developing professional skills in the field of instructional design and evaluate the design of the case event.
What you will do in the study:
You will answer 12 questions on the following web form.
If you choose to participate in the follow-up phone interview, you will be asked if you agree to an audio-taped phone interview following completion of the survey. All responses will be kept strictly confidential.
Time required: About 1/2 hour
Risks: None.
Benefits: There are no direct benefits to you of participating in this research study. The study may help us understand how the cases enhance the learning of instruction design.
Confidentiality: The information that you give in the study will be handled confidentially. Your information will be assigned a code number. The list connecting your name to this number will be kept in a locked file. When the study is completed and the data have been analyzed, this list and the audio-tape will be destroyed. Your name will not be used in any report.
Voluntary participation: Your participation in the study is completely voluntary.
Right to withdraw from the study: You have the right to withdraw from the study at any time without penalty.
How to withdraw from the study: If you want to withdraw from the study, email the case event co-coordinators requesting withdrawal.
Payment: You will receive no payment for participating in the study.
